Stem extension separation

I tried to raise my brand  new RAD mini 4 handlebars and have the extension separate and come off in my hands?  Wow that was scary. Wonder if the screw inside was loose or something else?  Anyone have this happen to them?

Maybe I'm undercaffeinated this morning, but what am I missing in your photo that shows a problem?  My Mini is an ST, but I think the handlebar assembly is nearly identical.

Are you saying that with the handlebar-height compression lock loosened, you were able to pull the handlebar stem up and completely out of the folding tube?  If so, I can't recall doing it, but I think that would be normal.  There might be a "max height" marking on the stem, just like the seat stem bar.
